Performance of an automated external cardioverter defibrillator for inhospital ventricular malignant arrhythmia

چکیده
Purpose Ventricular fibrillation (VF) and ventricular tachycardia (VT) are the major underlying rhythm during in-hospital cardiac arrest. For a patient in VF/VT the probability of successful defibrillation and subsequent survival to hospital discharge is directly and negatively related to the time interval between onset of the arrhythmia and delivery of the first shock. The data about this interval in clinical practice is heterogeneous and inconclusive, however the literature estimates it to be about 60 seconds in monitored units. Continuos ECG monitoring allows identification of such arrhythmias and alert nursing and medical staff. The time delay between the arrhythmic event and human intervention is still a challenge for clinical practice.

Universal Fluctuations in Very Short ECG Episodes
We propose a new algorithm for the detection of ventricular fibrillation (VF) in very short surface electrocardiogram (ECG) episodes. Ventricular fibrillation is the most commonly identified arrhythmia in cardiac arrest patients and can lead to syncope, within seconds. The fast detection of ventricular fibrillation is necessary for prompt defibrillation either with an implantable cardioverter/d...
متن کاملImplantable cardioverter-defibrillator in a patient with dextrocardia situs inversus
Background: Dextrocardia is a congenital anomaly, which may have coexistent coronary artery disease (CAD), arrhythmias and conventional indications for device therapy. However, the implantation of transvenous leads can be technically challenging and the approach needs to be tailored to the patient's individual anatomy. Case presentation: A 54-year-old male with dextrocardia situs inversus and i...
متن کاملThoracoscopic implantation of the implantable cardioverter defibrillator.
(Chest 1993; 103:271-72) T he implantable cardioverter defibrillator (lCD) has become accepted treatment for the management of certain malignant ventricular arrhythmias. ‘ Implantation ofthe lCD usually requires an anterolateral thoracotomy through which the patch electrodes and sensing electrodes are placed, in either an intraor an extrapericardial position, although subcostal and subxiphoid a...
متن کاملReuse Potential of Pacemakers and Implantable Cardioverter-defibrillators - Some Real World Data
Patients who survive either ventricular fibrillation or sustained ventricular tachycardia carry a high risk of further episodes, which could be fatal. Anti-arrhythmic drugs were the standard treatment for patients with malignant ventricular arrhythmias, but despite using the best appropriate medical treatment, arrhythmia recurrence rates remain high at 40-50% at five years. Implantable cardiove...
متن کاملIron-Sensitive Cardiac Magnetic Resonance Imaging for Prediction of Ventricular Arrhythmia Risk in Patients With Chronic Myocardial Infarction: Early Evidence.
BACKGROUND Recent canines studies have shown that iron deposition within chronic myocardial infarction (CMI) influences the electric behavior of the heart. To date, the link between the iron deposition and malignant ventricular arrhythmias in humans with CMI is unknown. METHODS AND RESULTS Patients with CMI (n=94) who underwent late-gadolinium-enhanced cardiac magnetic resonance imaging befor...
